Your GP has organised a case conference: a short, secure video meeting where the health professionals involved in your care plan it together. You're welcome to join — and to bring a family member or carer.
A case conference is a meeting between your GP and the other health professionals who look after you — specialists, physiotherapists, psychologists and others — all at the same time, on one video call.
Instead of letters going back and forth between clinics, everyone talks it through together, in real time — so nothing is missed, nothing is doubled up, and everyone leaves working from the same plan.

Your GPWhy you're invited
When several health professionals are involved in your care, your GP can bring them together to agree on one coordinated plan — or to check how the current plan is going. Because it's your health being discussed, you're welcome in the room.
You don't have to attend — the team meets either way, and your GP will always talk the plan through with you afterwards. But if you'd like to listen in, ask questions or have your say, the invitation is there.
Your GP will have discussed the meeting with you first — and you can change your mind at any time.
Your GP leads the conversation and knows your history. Just bring any questions you'd like answered.
Join for all of it, just listen, send a family member — or skip it entirely and hear the plan from your GP.
What to expect
We organise everything around you — the scheduling, the video link and the paperwork.
Studies consistently show that when care teams meet like this, patients' problems are picked up earlier, care plans work better, and hospital visits go down.
of Australian patients in a recent study believe case conferences had a positive effect on their care.
Cost & privacy
The meeting is covered by Medicare and bulk billed — there is no out-of-pocket cost to you, and nothing for your family to pay either.
The video call is encrypted, and only the health professionals involved in your care take part. Notes go into your medical record, just like a normal appointment.
The meeting only happens with your agreement. You can ask questions, bring a support person, or withdraw your consent at any time — just tell your GP.
The easiest place to start is your GP practice — they organised the meeting and know your care best. You're also welcome to contact the Conference.care team directly.
